For an Indian passport, a name change generally requires applying for re-issue of the passport with changed personal particulars. The exact documents depend on why the name is being changed.
The usual steps are:
Complete the legal name-change formalities and obtain supporting documents showing the new name. For a complete name change, Passport Seva may require supporting public/school documents issued in the changed name.
Apply for passport re-issue and select the option for change in personal particulars/name.
Submit the required documents, along with the existing passport and self-attested copies. Additional documents may be required depending on the circumstances.
Pay the applicable passport fee and book an appointment at a Passport Seva Kendra/POPSK.
Attend the appointment with the original documents for verification.
Police verification, if required, will be conducted according to the Passport Office's assessment.
After successful processing, a new passport containing the changed name is issued.
If the name change is because of marriage, divorce, remarriage, or death of a spouse, the document requirements can be different. For example, marriage certificates, divorce decrees, death certificates, or other prescribed declarations may be required depending on the case.
